What is the Scholarship Form?

The Scholarship form is the main registration form for camp. The health certificate (Section A of our scholarship form) must be filled out and signed by parent/guardian, and camper must bring health certificate to camp along
with the return statement (Section B) signed by parent or guardian.
The C & D sections of your scholarship form are to be filled out completely (type or print) and mailed to Alan Miller, Secretary, Conservation Camp, HC 32, Box 33, Upper Tract, WV 26866, before the June 1 deadline. The C and D sections can also be submitted online in the "Register for Camp" section link
